INSURANCES / PAYMENT                                                     

Our providers participate in most major health insurance plans. Below you will find a list of participating insurance plans. Please be aware that with any health plan, there may be deductibles and copayments for which you may be responsible.

Please note that we are currently only accepting new patients for Immigration Physicals.

​

If you have an HMO or Managed Medicaid plan, you must have Dr. Cespedes assigned as your primary care physician in order to be seen. It is your responsibility to call your insurance to assign Dr. Cespedes as your primary care provider. This must be done prior to your scheduled appointment, and not on the same day of your visit.  

 

We offer self pay rates for those without medical insurance.

We accept cash and all major credit cards, including American Express.

All Co-pays and Deductibles are due upfront at the time of the visit.

 

SEGUROS / PAGO

Nuestros proveedores participan en la mayoría de los planes de seguro de salud. A continuación encontrará una lista de particpating planes de seguro. Tenga en cuenta que con cualquier plan de salud, puede haber deducibles y copagos para los cuales usted podría ser responsable. 

 

Tenga en cuenta que actualmente solo aceptamos pacientes nuevos para fisicos de Inmigracion.

Si usted tiene un plan HMO, o medicaid administrado usted necesita tener la Dra. Céspedes asignada como su médico de atención primaria para asistir la cita. Es su responsabilidad de llamar a su seguro para asignar la Dra. Céspedes como su proveedor de atención primaria. Esto tiene que hacerse antes de su cita programada, y no en el mismo día de su visita.

 

Ofrecemos tarifas de pago por cuenta de los que no tienen seguro médico. 

Aceptamos dinero en efectivo y todas las tarjetas de crédito actuales, como American Express.

Todos los Copagos y Deducibles deben pagarse por adelantado en el momento de la visita.
