OFFICE VISIT SAFETY INFORMATION

1. Please inform the office of any symptoms of fever, cough, or shortness of breath via telephone prior to your arrival.  If needed, this will give us a chance to reschedule your visit to a telehealth session

2. Please notify the office if you have been exposed to someone with COVID over the past two weeks, or if you live with someone who has tested positive for COVID-19 in the past 4-6 weeks.  

3. Entering the office requires the use of a face mask or facial covering.

4. Please wash your hands or sanitize before and after visiting the exam room

5. We may take your temperature at the door prior to entering the office.

6. We need to limit non-patient visitors as much as possible.  Please make arrangements for child care and keep any family members at home if they don’t have an appointment

7. Please observe social distancing by sitting 3-6 feet apart from each other.

8. We may need to ask you to wait in your car or in the parking lot if the waiting area becomes overly occupied

6. Necesitamos limitar al máximo las visitas de personas que no son pacientes. Haga arreglos para el cuidado de los niños y mantenga a los miembros de la familia en casa si no tienen una cita.

7. Observe el distanciamiento social sentándose a una distancia de 3 a 6 pies uno del otro.

8. Es posible que tengamos que pedirle que espere en su automóvil o el estacionamiento si el área de espera está demasiado ocupada
